The CAP/C Program
What is CAP/C?
CAP/C stands for "Community Alternatives Program for Children". It is a program for medically fragile children under age 21 who are at risk of living in a nursing home. By providing in-home nursing care, case management, and other supports, CAP/C can help children stay comfortably at home.
Children who are eligible for CAP/C:
- Have a serious chronic medical diagnosis
- Require in-home nursing care related to the medical diagnosis
- Need to supplement other available supports in order to safely meet their needs.
- Must be approved for the appropriate type of Medicaid. (only the child's income is counted, not the parent's income)
Services
The Community Alternatives Program for Children (CAP/C)
The CAP/C Program provides medically necessary services, including in-home care, for children who may otherwise require long term hospitalization or care in a nursing facility.
In addition to in-home care, CAP/C particpants may receive the following services as well

Application Process
We are here to help guide you through
- A referral form is filled out by a local CAP/C Case Manager
- The referral information is forwarded to NC Medicaid, Division of Health Benefits who will determine if the child may be eligible for CAP/C
- Medical information is obtained from the child's physician. The information is then submitted to confirm whether the diagnoses and care needs meet a level of care appropriate for CAP/C
- A Medicaid application must be submitted for the child
- A Case Manager or case management team will visit the home to conduct an assessment. The case manager(s) will work with the family to develop a Plan of Care
- If the plan is approved by NC Medicaid, the Case Manager arranges the services with local community providers
- The Case Manager provides ongoing monitoring and coordination of care

We provide case management services for medically complex children, under the age of 21, in Durham, Edgecombe, Franklin, Granville, Halifax, Johnston, Nash, Orange, Person, Vance, Wake and Warren Counties.
