CAP/C is a program for medically fragile children under age 21 who are at risk of long term hospitalization or nursing facility care.
Your child may be eligible for nurse aide care, skilled care, home and vehicle modifications, specialized equipment, therapy services and more.
Applying can be a daunting task. Let one of our experienced Case Managers guide you through the process.

CAP/C stands for "Community Alternatives Program for Children". It is a program for medically fragile children under age 21 who are at risk of living in a nursing home. By providing in-home nursing care, case management, and other supports, CAP/C can help children stay comfortably at home.
Children who are eligible for CAP/C:
The CAP/C Program provides medically necessary services, including in-home care, for children who may otherwise require long term hospitalization or care in a nursing facility.

We provide case management services for medically complex children, under the age of 21, in Durham, Edgecombe, Franklin, Granville, Halifax, Johnston, Nash, Orange, Person, Vance, Wake and Warren Counties.
Our team of social workers and registered nurses understand that not one care plan fits all. We will take the time to get to know your child and family to develop a person-centered plan that fits your child's specific needs.
